Why we need to do a usage survey.

"The Chicago Park District requires that usage surveys of the proposed area should be conducted for a period of eight months to a year. These surveys determine the existing use of the proposed D.F.A. site and identifies who currently uses the site and what types of activities they are participating in, which days and hours the park is heavily used, weather conditions, other amenities around the site such as a playlot or athletic field, and if the surveyed is a dog owner." *

Even though this land is unused and isn't already an official park these procedures still need to be followed since this land is managed by The Chicago Park District .

After you go to Michigan & Roosevelt and fill out form follow instructions below to enter data online.

Instructions To Enter Usage Survey Data Online:

1. Connect
Connect
to Yahoo!

After you select the link above a screen will say:

"Oops...
You must be signed in to access the group dogpac_il.
To sign in, please click here: "Sign In"

2. Sign-In
Select "Sign In"
which will bring you to a page that gives you the following log-in choices:

If you are NOT a Yahoo! member, you must fill out free registration first and get a "Yahoo! ID" and "Password".

OR

If you ARE already a Yahoo! member, fill in your "Yahoo! ID" and "Password".

3. Join "dogpac_il"
In the upper right corner of your screen select the "[Join This Group!]" link. Answer a few questions and select the "join" button at the bottom of the screen.

4. Record Data
Once you have successfully logged in add your usage survey data:

(a) select "Add Record"
(b) fill out the fields
(c) select "Save Record".
